چکیده
Introduction: Multiple myeloma is characterized by plasma cell infiltration of the bone marrow and presence of a monoclonal protein in plasma and/or the urine. Multiple myeloma with biclonal gammopathy is rare (1%). Case Report: In this report, we describe a case of biclonal multiple myeloma having two M bands on serum protein electrophoresis. He had elevated serum IgG and IgA levels. Ig G and IgA with Kappa and Lambda light chains respectively were detected by immunofixation. It is a very rare immunofixation pattern seen in any gammopathy. The patient presented with vertebral and pelvic bone involvement and neurosensory involvement of both the lower limbs. Patient has been lost for follow up at present. Conclusion: It is a very rare form of multiple myeloma accounting for 1% of all cases. Clinical presentation and response to therapy in such cases appears to be like any other multiple myeloma cases. They should be treated like any other multiple myeloma case.

Biclonal Gammopathy in Chronic Lymphocytic Leukemia: Case Report and Review of the Literature.
Monoclonal gammopathies are frequently seen in B-cell malignancies. Monoclonal proteins are seen in a significant proportion of patients with chronic lymphocytic leukemia (CLL), which is a clonal disorder of mature B cells. The use of more sensitive laboratory methods has enabled the detection of monoclonal proteins or light chains in the serum and/or urine in the majority of these patients. Th...
متن کاملA Rare Occurrence of Plasma Cell Myeloma with Biclonal Gammopathy
Plasma cell myeloma is known to cause expansion of a single clone of immunoglobulin (Ig) which results in the secretion of a unique homogeneous monoclonal protein (M component). However, there are cases which reported that it can also cause production of two different clones of these monoclonal M proteins. Although it is relatively very rare as the prevalence is only 2% of all plasma cell myelo...

Scleromyxedema is a rare chronic cutaneous mucinosis usually associated with a monoclonal gammopathy and underlying systemic disease. The etiology of the disease is not known. There are no standard treatments and response to various therapeutic modalities varies. We report a case of refractory scleromyxedema in a 63-year-old man with a biclonal IgG and IgM λ-gammopathy. The patient was successf...

We reported a rare case of pure red cell aplasia in a 44-year-old man with multiple myeloma with biclonal gammophathy (IgG lambda and IgA lambda type) with severe anemia. After treatment with bortezomib, adriamycin, and dexamethasone, the patient achieved very good partial response with disappearance of monoclonal gammopathy. However, his anemia was not significantly improved. Bone marrow analy...

We report a rare case of multiple myeloma with biclonal gammopathy (IgG kappa and IgA lambda type) in a 58-year-old man with prostate cancer who presented with lower back pain. Through computed tomography (CT) imaging, an osteolytic lesion at the L3 vertebra and an enhancing lesion of the prostate gland with multiple lymphadenopathies were found. In the whole body positron emission tomography-c...
